EN סיפור פיסטולה פריאנלית (עם ניתוח בעבר ע"פ אנמנזה בלבד) ראומטולוגי במעקב IBD לספונדילוארטרופטיה משנית ל חשד בטן 6/2011: פיתול מעי דק באגן בעל דופן מעט מעובה CT קולונוסקופיה 9/2011: היצרות בעומק 40 ס"מ מפי הטבעת ללא יכולת, בביופסיה דלקת כרונית קלה בלבד קולונוסקופ ילדים העברת נסיונות חוקן בריום להדגמת המשך המעי הגס כשלו עקב רגישות וכאב

3 פרשיית החולה בקורסים של סטרואידים ורפאסל. טופלה 3-6 X - תחת טיפול באימורן 150 מ"ג כאב בטן ניכר ושלשול ביום. בבדיקתה: בעודף משקל, ללא ממצא בבדיקה הגופנית מגבול עליון, 1.5 X מוגבר CRP ש"ד מוחשת, ללא אנמיה או במעבדה: וויטמין B12 D מתחת לנורמה קולונוסקופיה: 35 : 3/2012 ס"מ מפי הטבעת קטע קצר של רירית דלקתית ומעבר בזוית קשה, שאר המעי הגס וכן המעי הדק הסופי תקינים. נמצאו :CTE 7/2012: היצרות הסיגמא במקטע קצר עם דופן מעובה והרחבה פרוקסימלית ללא הסננה מסביב. הרחבה קלה של פיתולי מעי דק בבטן, איליום סופי תקין. התחתונה

4 אי התאמה ניכרת בלטה ממצאים אוביקטיבים דלים לפעילות מחלה: הסבל הניכר של החולה: במהלך כחצי שנה פנתה לחדרי המיון השונים ב 4 חולים עשרות פעמים, בתי וכמובן פניות חוזרות מרובות למרפאה עקב כאב בטן ניכר, והקאות. היצרות בסיגמא הדיסטלית עם דלקת קלה באזור זה, ללא עדות למחלה מוקוזלית נוספת, ללא אנמיה ומצב תזונתי טוב ללא ממצא משמעותי לציין בדיקות מעבדה חוזרות ללא שינוי : CRP מוגבר 1.5-2X מהנורמה, ללא אנמיה, השאר תקין. בדיקות חוזרות בחדרי המיון כולל לפחות 2 סדרות צילומי CT בטן

7 פרשיית החולה במחשבה על בעיה מכאנית עקב ההיצרות במעי הגס בוצע לניסיון הרחבה. הקלה, ונשלחה : ללא טיפול בלקטולוז ניסיון צירקולרית בקולונוסקופיה 10/2012: עומק 25 היצרות באורך 3 ס"מ, בה רירית בצקתית ודלקתית ובשוליה נראו מ ניצפה איזור עם דלקת קלה סס" בעומק סעפם סעיפים. גם של הרירית, ן, תקין השאר כולל TI הסגמנט המוצר לא נמצא מתאים להרחבה.

9 פרשיית החולה הועלתה השאלה האם הממצאים הדלקתיים המקומיים הינם חלק ממחלת קרון או משניים למחלת סעיפים, לציין שזו הקולונוסקופיה הראשונה מתוך ארבע בה דווח על נוכחות סעיפים. בוצעה רויזיית הפתולוגיה מהעבר: בלתי ספציפית. ממצאים של דלקת הוחלט להפסיק את הטיפול ביומירה ובאימורן.

10 פרשיית החולה במחשבה על הצרות מכאנית כגורם לכאב המוצר. כריתת האזור הוחלט על לאחר הפסקת הטיפול שבועות לניתוח, 4 בהמתנה האימונוסופרסיבי אושפזה שוב עקב כאב בטן וממצא של אבצס באגן. היצרות בגובה באופן שמרני קובסיגמואידוסקופיה: טופלה ס"מ עם דלקת קשה באזור.

11 פרשיית החולה (10 שבועות לאחר האשפוז) 2/2013 החולה עברה ניתוח אלקטיבי בו נמצא: המעי היורד כולו מעובה המעי הדיסטלי והסמוך לסיגמא חולה ביותר עם עיבוי הדופן וטיפוס שומן אבצס באזור הסיגמא שנוקז, פיסטולה מהסיגמא לחצוצרה עקב מצבו הרע של המעי הפרוקסימלי לא ניתן היה לבצע השקה ובוצעה עע"ש הרטמן וקולוסטומה וקולוסטומיה פרוצדורה

14 Crohn s-like reaction in diverticular disease The files of the Department of Histopathology of the General Infirmary at Leeds were searched for specimens coded for both Crohn s disease and diverticulosis or diverticulitis. Over a 20 year period ( ) 11 cases were identified. Symptoms persisted, and eventually a colonic resection was undertaken Gut 1998;42:

15 In all cases: the gross description included mural thickening; creepage of fat was mentioned in some, but not all. In all cases there was hypertrophy of the muscularis propria, with diverticula and peridiverticular abscesses typical of diverticular disease. The mucosa was frequently described as edematous or cobblestone in appearance.

16 In all cases, microscopy revealed transmural granulomatous inflammation and ulceration typical of Crohn s disease, with crypt abscess formation and fissuring ulceration in most cases. The granulomas were present throughout the wall and in serosal fat. They appeared to be true granulomas.

17 Treatment and outcome Three patients underwent proctocolectomy, The remainder were treated by sigmoidectomy, Follow up varied from one to 15 years. In the study group, there were two perioperative deaths. In 8 of the 9 remaining patients there were no recurrence of IBD. in this patients, the histological feature of classic Crohn s disease was an incidental finding in a clinical setting otherwise typical of diverticular disease.

19 A systematic review by use of multiple health care databases was conducted looking for: IBD, colitis, UC, Crohn s disease, diverticulitis, diverticular associated colitis, segmental colitis, and segmental colitis associated with diverticular disease. Reports containing clinical data such as age of onset, treatment, recurrence of symptoms, and follow-up time of patients with the diagnosis of DAC based on accepted clinical, endoscopic, or histologic findings. The clinical symptoms of tenesmus, hematochezia, diarrhea, bloody diarrhea, abdominal pain, and nausea. Endoscopic findings including focal erythema, submucosal ecchymosis, erosions, and ulcers. Histologic criteria of luminal inflammation in biopsy or resection specimens.

20 227 participants were selected from 18 eligible studies. The average age of disease onset was 64 years. three-quarters of patients (163/227) from eligible studies were classified as having DAC, whereas, of the remaining one- quarter were patients had CD overlapping with diverticular disease or patients with focal active colitis in an area of diverticulosis.

21 DAC may respond well to medical therapy with 5-ASA 28 patients of 163 (18%) eventually required an operation: 25 segmental colectomy; 3 total colectomy. Six studies tdi reported td progression from DAC to overt IBD.

22 Four studies described patients who had pathologic and clinical manifistations of diverticular disease that resembled CD. The clinical and pathologic overlap of DAC with IBD makes the differential diagnosis critical because misdiagnosing the patient s disease can lead to medical mismanagement.

28 SCAD segmental colitis associated with diverticula SCAD is a form of chronic colitis limited to areas of the colon with diverticula and sparing the rectum. The histological features of SCAD bear close resemblance not only to idiopathic IBD. Cryptitis, i crypt abscesses, and even granulomas and chronic architectural distortion are described in patients with SCAD.
